Sliding Fee Discount Program
Fountain Health Care LLC is committed to providing high-quality behavioral health services to all individuals, regardless of their ability to pay.
We offer a Sliding Fee Discount Program (SFDP) for eligible patients based on household income and family size, in accordance with the most current Federal Poverty Guidelines (FPG).

Fountain Health Care LLC does NOT deny services to any individual based on:
- Inability to pay
- Insurance status
- Enrolment in Medicare, Medicaid, or CHIP
💲 Sliding Fee Discount Eligibility
Patients may qualify for discounted services based on their income level:

| 0% – 100% | 100% discount (Nominal fee only) |
| 101% – 150% | 75% discount |
| 151% – 200% | 50% discount |
| Above 200% | No discount (Full fee applies) |
💳 Nominal Fees (For Patients at or Below 100% FPL)
Patients who qualify at or below 100% of the Federal Poverty Level will pay a nominal fee, which may include:
- Initial Assessment: $0 – $20
- Individual/Group Therapy: $5 – $30 per session
- Medication Management: $10 – $30 per visit
Nominal fees are designed to be affordable and will not be a barrier to receiving care. Fees may be reduced or waived based on individual financial hardship.
